What the Unit expects to see in a complete file.
Core forms
Application form and disclosure form
Form 12, photograph and signature certificate
Medical questionnaire, including HIV test
Investment confirmation form
Supporting documents for each applicant
Certified copy of the passport biographical page.
Original or certified birth certificate.
Marriage certificate, or divorce decree, where relevant.
Police certificate from every country of residence over the last ten years.
Proof of address dated within the last three months.
Six passport sized photographs taken within the last six months.
Bank reference and evidence of the source of funds.
Professional reference and curriculum vitae for the main applicant.
Formalities
Documents not in English must be translated by a certified translator and the translation attached to the original.
Documents issued abroad generally require an apostille.
Nothing older than six months is accepted for police certificates, bank references or proof of address.
